Abstract

In general, the ‘line of force’ in the labial technique is better defined and understood than the lingual orthodontic technique. The variability in vertical positioning and profile thickness of the lingual brackets makes lingual biomechanics more difficult to evaluate.

The difference in relationship of the point of force application to the center of resistance requires special understanding and clinical application for successful clinical results.

Whilst space closure and space regaining procedures are associated with different moments acting on the system leading to different patterns of bowing, interpretation of the anchorage value is also different in lingual than labial orthodontics.

Clinical procedures like choice of extraction, distalisation, re-proximation and expansion needs different planning and approaches in the lingual technique compared to the labial technique.

Similarly, extraction space closure and planned anchorage loss in the lingual technique has to be performed cautiously keeping adverse force moments in check.

Apart from biomechanics, special clinical situations like periodontally compromised dentitions or prosthetic rehabilitation cases require an understanding of the strength and weakness of the lingual technique.

This presentation is a thorough insight on various biomechanical challenges and their clinical solutions for space management in lingual orthodontics – which can be referred as “The Protocol for Space Management in Lingual Technique”.

Dr Surya Kanta Das, presently the President of the Lingual Orthodontic Society of India, is a Titular member of ESLO (European Society of Lingual Orthodontics) and an active member of WSLO (World Society of Lingual Orthodontics). He is the chief faculty of the Lingual mini-residency program held at SCB Dental College and faculty at the IOS-IDA fellowship program.

Dr Das, an eminent speaker at national and international forums is credited for introducing the modified Hiro’s technique, DTC device for lingual customisation, DLAP software and D-Mushroom arch forming pliers. Presently he is working as Professor and Head, Post Graduate Department of Orthodontics, SCB Dental College.
